Picture of drivers license for each driver. Clearly readable image of front of license. No larger than 1 megabyte (1mb) and in .jpeg / .jpg / .png format. WARNING - if you take a picture with your phone it will likely be of a very high resolution and too large to upload. Such images should be scaled down prior to submission. We recommend you format your images prior to completing this form.
